I don't really know anything The Coffin of Andy and Leyley, but the Steam reviews and the screenshots indicates that the game sexualizes and fetishizes the incest. The "hot goth girl but she's your sister" ordeal and "they may be having sex behind the scenes" themes.
I don't think it's a big deal though, primarily because it's not something that I personally find super offensive, but that could just because I've spent too much time on Gelbooru and have become mostly immune to (fictional) incest.
It's very obvious that The Coffin has brother-sister incest for fap material and sexual fantasy reasons. It's obvious that it was included to make people into incest horny as they play the game.

That I agree with you there, but you also have to remember that we're likely desensitized to this kind of smut, so it may be objectionable to people who aren't familiar or used to seeing incest out in the open.
I bet 9 of 10 these people get their rocks off shipping Winchester brothers from Supernatural because I doubt this outrage is real.
