To be honest I barely understand the point of this one. I think essentially she's being kept in some cruel and unusual captivity where the only way she can both pee and drink is by riding the dildo and its connected mechanisms. Once she's peed enough, the feeder up top will feed her some more drinking water.

Catheters don't feel good, it's different than sounding, it just feels like you a got a big uncomfortable stick in your piss hole, and you can't control your piss anymore it just constantly leaks out. Some people might get turned on by that but for the majority of the population it's unpleasant.
Forcibly drinking water was a common form of torture in the past, so this just combines the two into a "YOU GOTTA STAY HYDRATED!" kind of nightmare.

First off this is not one of the artist's "Saw But With Sex" predicament bondages where the victim must complete a specific task or suffer a penalty such as impregnation before being released. The title implies that this is a straight-up torture device and the girl is a warrior who has been captured and imprisoned and will be stuck here until her captors decide to free her.
As near as I can tell it works like this:
