I mean most things on there, not really into guro though but some devs tend to put in as much as they can. Sinister is quite guro-heavy but just dont lose to the wrong monsters.

Claire's Quest(rpg beastility, rape, straight shota, shotadom, monster birth)
Goblin Burrow(strategy, rape, loli, pillage village to make an army)
Goblin Walker( rpg, capture your victims etc.)
Lonarpg (rpg, do missions, story bad end good ending, not-safe edgy jokes, monsters etc.) - you'll probably need a mtl to translate the latest update.

I wish I knew of some good hentai games to recommend, but unfortunately I don't.

Koikatsu and Honey Select are the only hentai games I kept around for more than 30 minutes.

Also, how does anyone not into Guro manage to deal with hentai games with guro? If it gets too disturbingly gory for me, I'm out.

You get use to it.

I've gotten used to Mortal Kombat, but I don't think I could get used to something like that in a game designed for sexual gratification.

The game would have to be very enticing and the gore would have to be either non-detailed or exceptionally rare to encounter.
