I think the hand manacles are connected to her neck chain. That way, when she tries to lower her hands to her face, the chain starts to pull on her neck. if she wants to touch her own breasts, she'll have to be standing perfectly straight, and any lower than that and she'll start choking herself.

Because of this, she cannot sit down or lie down normally. She might be able to get on her knees with her back and thighs straight, and let her arms take the weight, and that's as comfortable as it's going to get.
