My interpretation is that the blue-haired woman is kneeing the other in the stomach, which produces vomit (and hurts like hell). Her left hand is pressing her victim's throat, impeding her breathing, and her right hand is controlling her victim's head. That leaves only her mouth to seal the brunette's mouth shut. What's unclear is what the next step is. Since the victim's cheeks are bulging, the blue-haired woman may just want to trap it all in the other mouth and make her hold on to it. "I'm going to injure you, make you vomit, and then compound it by not letting you get rid of it." Or maybe she just doesn't want it all over herself.
