Because the series this image is based on has heavy guro content. Yet another of those situations where whoever uploaded this ignored the "Tag What You See, Not What You Know" rule.

"Tag What You See, Not What You Know". Yes, much like many other tags, the meaning of "guro" here is the signified that is internal to the individual image which occurs on the same web page as said signifier. "Guro" is implicit here in the knowledge of Uzumaki; Uzumaki is partially guro.
