Engineer's creepy face could be a reference to a feature of the HWM model of the original Engineer, and if I'm right, then the artist really knows his TF2.

HWM model Engineer can have his goggles removed, allowing you to see his eyes. When that happens, you'll see a frightening engie stare. Youtube "Staregineer" and see for yourselves.
